What to do next if my sperm count is low?

Your next step should be to evaluate levels of hormones like testosterone, LH, and FSH and MRI of brain

Testosterone

Low sperm count may be due to low or absent testosterone levels

Total

Total testosterone should be measured between 8 and 10 am in th mornings only. The value changes during different times of the day.

Free Testosterone

Free Testosterone should also be tested from the same sample. It is the component of testosterone that is freely circulating in the blood.

Pituitary Hormones

FSH, LH and Prolactin are the other three hormones whose levels should be checked

FSH

Fsh or (Follicle Stimulating Hormone). If its concentration is high, combined with a low testosterone value, then it points to a testicular/genetic cause.

LH(Leutinizing Hormone)

If its value is high together with a low testosterone value it means that the testes are not producing testosterone in response to stimulation from the pituitary in the brain.

Prolactin

An excess of this hormone can interfere with testosterone production. It may be increased due to an overgrowth in the pituitary gland in the brain.

MRI Brain

This may be advised to check for overgrowth in the pituitary glands
